Patient Access - Online Health Record Viewing System (EHR)

What is the Online EHR Viewing System?
The system is a website that allows you to view your medical record over the internet from a PC. It allows you to easily and quickly view the electronic medical information held about you by The Vale Medical Centre.

The information is presented to you in a format that is easy to navigate and offers you links to resources such as patient information leaflets about diseases, tests, investigations, support groups and medications etc. There are also links to websites such as NHS Direct Online and Patient.co.uk where you can find additional information to help you understand and educate yourself about what you read in your health record.

Signing Up

Simply go to http://www.patient.co.uk/patient-access.asp to find out information about the service.

If you want to register you can do so by selecting the Green Register button.

When asked if you have received a registration letter from your practice, answer:  “NO

When prompted for a postcode, enter: “SE23 2JF

You will be led through questions to help you get online access to your records. 

Once registered, if you have a phone running Android or IOS you can use your device to make appointments or order medication by downloading the app from the store. Searc for "Patient Access" in the App Store

About Patient Access

Patient Access is only available to patients who have signed up to use it at The Vale Medical Centre.

Using Patient Access, you can now view your medical records from home, work or on the move - wherever you can connect to the internet. What's more, because Patient Access is a 24 hour online service - you can do this in your own time, day or night.

Confidentiality and Security

Information sent via Patient Access is encrypted.  This means that messages sent cannot be intercepted or read by others, and only the Patient and the Practice are able to see any personal information.

The computer system is connected to Patient Access through the NHS network, which is more secure than a normal internet connection.  The Surgery will only enable the internet access facilities if requested to do so by the Patient. 

Terms and Conditions

Whilst the Surgery makes all reasonable efforts to provide the Service, it is not liable for any failure to provide the Service, in part or full, for any cause that is beyond its reasonable control.

This includes, in particular, any suspension of the Service resulting from maintenance and upgrades to the systems or those of any party used to provide the Service.

You must keep your Personal Details secret and take all reasonable precautions to prevent the fraudulent use of your Personal Details.  If fraudulent use is suspected, contact the Surgery as soon as possible.

The Vale Medical Centre reserves the right to change the service from time to time and shall give appropriate notice of any material changes.  They may, where considered appropriate for patient protection, suspend, withdraw or restrict the use of the service or any part of the service.  Patients will be notified as soon as practicable if any such action is taken.  The surgery reserves the right to vary these Terms and Conditions and appropriate notice will be given of any material changes.


Booking Online Appointments

Using Patient Access you can see what available appointments there are, then book an appointment to see the doctor.  There is a limit of one appointment per patient.  Please bear in mind that if you already have an appointment booked on our system you will be unable to book further appointments.  

Cancelling Online Appointments

Using Patient Access you can cancel appointments that you may have.  When logged onto Patient Access you can see what appointments you have booked, who it is with and at what time.  If you are unable to keep your appointment please cancel your appointment either by the online method or by calling the practice.  We reserve the right to exclude access to online booked appointments if you fail to attend your appointment or do not cancel it in time.

Doctor appointments are always in demand and so we can continue to offer a good service we require all patients to cancel their appointments (whether booked over the telephone, in person or online) in good time so that it might be offered to someone else who needs to be seen.
